► Along with vaccination and treatment, testing continues to be an important tool to manage the COVID 19 pandemic. Family Care Associates wants to ensure testing resources remain available to the public.
On July 7th from 11:30am - 1pm we will be having a drive-thru clinic to offer at home COVID tests FREE of charge. You DO NOT have to be a Family Care patient to receive these kits.

Family Care has several providers available to take over your medical care. Please contact our office to select a new Primary Care Physician of your choice or if you would prefer to change practices, our staff will assist you in transferring records to your new provider.

Family Care Associates will NOT have late night hours until further notice.
In an effort to protect our healthcare providers and staff, we ask that patients with a fever, cough or shortness of breath call our office in advance. Do NOT report to the office or the ER without calling ahead of time.
Our team of nurses will triage symptoms to determine the best course of action. Individuals that are experiencing only mild symptoms may be asked to self quarantine at home.
Patient's with respiratory symptoms will be triaged in the back entrance of the building. Patient's that require further assessment will immediately be given a mask to wear before entering the building.
Please keep in mind that resources and various testing may be limited. Not all individuals experiencing symptoms will need further testing. Testing will be determined by age, past medical history and severity of symptoms.
Social distancing is of utmost importance. Please stay home when at all possible.

► Team-based care is a physician led team of health care professionals that work together to achieve mutual goals, increase patient satisfaction, and improve overall health outcomes. The team of professionals will consist of a Primary Care Physician, 2 Nurse Practitioners, a Nurse, a Medical Assistant as well as other ancillary members such as Care Coordinator, Clerical Staff, Phlebotomist, Licensed Clinical Social Worker and/or Radiology Technician.
