Established in 1990, Family Care Associates is an independent, physician-owned medical practice serving Effingham and the surrounding communities. We provide comprehensive healthcare services for patients of all ages and are committed to delivering high-quality, personalized care close to home.
Family Care Associates is a Certified Rural Health Clinic and an NCQA Patient-Centered Medical Home. We are proud to offer an integrated healthcare model, bringing together primary care providers and behavioral health professionals to support the whole person. Our team includes physicians, nurse practitioners, nurses, and four Licensed Clinical Professional Counselors who collaborate to provide coordinated care that addresses both physical and emotional well-being.
